Plan Your Visit With Us
Our office is located right in Cobbs Corner in Canton, MA! Our office is in the Office Suites which can be found between Chico’s and Edible Arrangements. We have an elevator located at the main entrance which will bring you up to the second floor, where we are located.
- Please plan on your appointment to be at least 1.5 hours long. The appointment may take a shorter or longer time.
- Bring your eyeglasses with you
- Bring your insurance cards (if you have more than one bring all of them).
- If your insurance requires you to have a referral, please call your PCP for one, prior to your appointment.
- We will be dilating your eyes for the retina eye exam. This can make it difficult to see afterwards. The dilation usually lasts for a few hours. If you have sensitivity to dilation we do recommend you bring someone to drive you after your exam. Please note we do dilate patients’ eyes every time they come in for an exam.
If you would like to fill out the new patient forms beforehand, please print the “New Patient Packet” and completely fill it out.
Don’t forget to bring it to your appointment! We also have the packets printed at the office, where you can fill it out after checking in.
